I am currently in network with some insurance companies. Upon your request, I can provide you with a list of fees and billing codes and/or a completed statement with codes necessary to file an out of network claim with your insurance carrier, if you choose to do so.  I cannot guarantee that your insurance carrier will approve your claim. So, please call them before scheduling an appointment. I am in the process of getting credentialed with insurance provider so stay tuned and the page will be updated once that information is finalized.

New patient appointment (60 minutes): $375

Standard follow up appointment (25 minutes): $180

Extended follow up appointment (50 minutes): $210

Prescription Refills outside of appointment: $35

*Not allowed if a patient has not been seen for more than 3 months.

Paperwork not completed during session: $50-$100 depending on amount of time needed

Cash, credit card, Zelle, or Venmo are all accepted forms of payment.
